NSW Medical - Recording an Individual Health Support Plan for a Student

When a student has a medical condition that requires a detailed response it is vital that the information required is readily available.   Because of this, the DoE has requested that all SMS's use the same terminology when recording and managing medical action plans.

A medical action plan is to be referred to as an Individual Health Support Plan.   Hero has devised a way for staff to easily identify students with a IHSP and to quickly access the required information.

UPLOADING AN INDIVIDUAL HEALTH SUPPORT PLAN

1. Find the student using one of the search tools available

2. Click on the student Information (i) icon

3. Click on the WELLBEING Section heading.

The section you are in will be displayed in blue and underlined. The fields for that section will be displayed.

4.  IHSP plans need to be uploaded to the Individual Health Support Plans page.   This page will only show in the student menu if the Individual Health Support Plan Uploaded? field is ticked prior.  This field appears under a student's medical conditions.  Ticking this field will also add the student to the autogroup of the same name.

5.  You will now see a newly visible page.   This page is attached to a Page Set so will only show if a student has 'Yes' ticked in the above field.

6.  Before adding the post, click the 3 dots icon > About this Page to read the DoE provided information.  This contains a link to the Individual Health Support Plan Cover Sheet.  

7.  Click Share something to create a post .  Click ADD FILES or EMBED FROM DRIVE to upload/attach the required document.  Click NEXT.

8.  Tags have been created to further identify the type of document added.  Click the required tag followed by SAVE POST.

IDENTIFYING STUDENTS WITH AN INDIVIDUAL HEALTH SUPPORT PLAN

Hero has created an autogroup that will auto-populate when the Individual Health Support Plan Uploaded field is clicked on a student.

1. Click the group icon (2 heads), followed by AUTO to locate the autogroup.

2. Click the autogroup to see the students with the Individual Health Support Plan Uploaded field clicked.

3. To further identify the students, this autogroup has a dedicated icon which will show on any students in this group.  This icon is visible in any grid search.

4. Tag Data Sheets can also help identify students with particular IHSP plans.  Go to Quick Links > Tag Data Sheets and click INDIVIDUAL HEALTH SUPPORT PLANS.  

5. Click the piece of the pie in the graph to see what students have what plans.
